How to Apply

The Foundation reserves the right to limit financial contributions to those organizations that meet the following criteria, as determined within the sole discretion of the Foundation. The Foundation also reserves the right to monitor and review the use of funds contributed to a project or organization.

Grants Under $5,000

For grant requests under $5,000, the following information should be submitted when requesting funding, including the following:

A cover letter containing all institutional information, including an address and contact information (phone numbers, fax numbers, e-mail addresses).
A brief statement describing the purpose of funding, the funding goals and the need for financial assistance. This statement should not include technical or scientific terms, but language suitable for laymen to comprehend goals.
Provide as much detail as possible about the project for which funds are being sought such as: scientific descriptions; bibliographies; proposed budgets; and anything else that would assist the decision of the Foundation.

Grants Over $5,000

A proposed project must be consistent with the mission of the Foundation. The following types of tax-exempt organizations and/or endeavors are eligible for receiving an award from the Foundation:

Medical or health care delivery
Scientific
Disaster relief
Education
Subsidiary-specific, local cultural events
Philanthropic activities in our medical therapeutic categories
Local youth activities (exclusive of sport teams / events)
Environmental and public safety

For grant requests exceeding $5,000 the applicant must complete a Grant Application Form.

Upon receipt of the request for a financial contribution the Foundation Grant Review Committee will promptly review each request and will endeavor to notify the applicant of the outcome within thirty (30) days.

Prior to final approval each applicant requesting a grant exceeding $5,000, must complete a Grant Agreement Form.
